Initial Visit with the Psychiatrist

1. When you arrive in the waiting room, please turn the light switch on that is next to my name.

2. Before coming to your first session, please go to the section of this website entitled "Forms and Questionnaires" and download all of the forms, fill them out, and
PLEASE BRING THESE FORMS WITH YOU TO THE FIRST SESSION. If you have forgotten to do so, I will need you to complete them during this session.

3. Also under the page "Forms and Questionnaires" there are several questionnaires to help us establish some of your problems. Please view, download and fill-out the ones that seem relevant to you and bring these to the visit as well.

4. Please bring copies of any relevant testing or medical records for your medical file at my office.
