Professional Fees and Services

 

If you have insurance, you will be provided with an insurance statement to submit to your insurance company.

Patients are responsible for payment of account regardless of insurance coverage and elgibility.

I accept cash, check, Visa or Mastercard.

A credit card will be held on file to guarantee payment but will not be charged without notification

** Includes oral or written communication with patient, parent, school, therapist, physician, insurance, or any other entity deemed necessary with permission and record-keeping of said communication.

 

Telephone and electronic communication and the documentation of the services provided involve Dr. Werner-Crohn’s time.  By utilizing these convenient options, these services may replace or prevent the need for an office visit.  Therefore, telephone and electronic care are recognized as patient care by the American Medical Association and some insurance providers, and you will be charged as per the fee schedule above.
